Auction Donation Form

Please make your donation before March 1 to be included in the catalog.

Be sure to include your phone number in case we have questions about your donation, and
remember to hit the submit button at the bottom of the form when you are done.

You should see a confirmation screen after you hit the submit button summarizing your donation.
If you need to make corrections, just resubmit with a note in the description box or send an e-mail to webmaster@olyuu.org.

Donated items will be collected at the church between services on March 15 and March 22.
